Most insurers require
patients to make a co-payment for products and services. Therefore, your rehabilitation equipment
and services may have co-payments you are responsible for, just as when you
visit a physician.
If you have received
a statement from OrthoRehab on a co-payment or self-pay bill, you may submit a
payment with a credit card by contacting one of our collectors. You will need your invoice or patient
account number. We accept Visa,
MasterCard, and Discover Card.
If you would prefer
to submit a payment by check, please send your check to: OrthoRehab Attention:
Co-Payment Collections 1415 W. 3rd Street, Suite #101 Tempe, Arizona 85281. To ensure that your money is credited to
the correct account, please include your invoice or patient account number.
